In consideration of participating in the Miss Georgia USA and Miss Georgia Teen USA program and its related events and activities, I agree that:

1. There are inherent risks of injury.

2. I knowingly assume those risks and agree to indemnify and hold harmless, the Miss USA/Teen USA System and Greenwood Productions, Inc. for all injuries sustained.

3. I am soley responsible for all the items I bring to the pageant. Any item lost or stolen is not the responsibility of Greenwood Productions, Inc.

4. I am soley responsible for any damage I may cause to my hotel room or the pageant show venue.

I have read this release of liability and assumption of risk agreement, fully understand its terms and sign it voluntarily. I understand that if I am under the age of 18 I must have a parent or guardian sign this form on my behalf.
